Overview

Clinical Notes provide structured documentation of patient encounters. Bio Ecko supports SOAP (Subjective, Objective, Assessment, Plan), free-text, and template-based note formats. Notes are stored in the EMR and form the core of the medico-legal record.

SOAP Notes

The SOAP format structures clinical documentation:

  • Subjective -- Chief complaint, history of present illness, review of systems (auto-populated from intake forms if used)
  • Objective -- Physical examination findings, vitals (auto-pulled from vitals entry), lab results summary
  • Assessment -- Diagnosis (with ICD-10 code picker and AI-assisted suggestions), differential diagnoses, clinical impression
  • Plan -- Medications (linked to prescription module), investigations ordered, procedures planned, follow-up instructions, referrals

Each section supports rich text, voice-to-text dictation, and quick-phrase shortcuts.

Templates

Speed up documentation with clinical note templates:

  • Pre-built templates for common presentations (Upper respiratory infection, Diabetes follow-up, Hypertension review)
  • Specialty-specific templates (Ophthalmology examination, Orthopedic assessment, Psychiatric evaluation)
  • User-created personal templates
  • Shared department templates managed by the HOD
  • Template variables (auto-fill patient name, age, vitals, last lab values)
  • Smart templates that show/hide sections based on diagnosis selection

Voice-to-Text & Smart Shortcuts

Reduce typing with:

  • Voice dictation with medical vocabulary recognition
  • Quick-phrase shortcuts (type '.bp' to expand to 'Blood pressure: / mmHg')
  • Auto-complete for diagnosis names, medication names, and procedure names
  • Copy-forward from previous visit with smart differencing
  • Batch entry of examination findings using checkbox-style rapid entry forms

Addendums & Co-Signatures

Maintain note integrity:

  • Once signed, notes cannot be edited -- only addendums can be added
  • Addendums are timestamped and linked to the original note
  • Co-signature workflow for resident notes (attending physician reviews and co-signs)
  • Late entry documentation with automatic late-entry flag
  • All modifications tracked in the audit trail

Tip

Create 5-10 personal templates for your most common encounters. This can reduce documentation time by 60%.

Warning

Signed clinical notes are medico-legal documents. They cannot be deleted or silently modified. Always use the addendum feature for corrections.

Clinic tip

Solo doctors should use the Quick Note mode which combines all SOAP sections into a single text area with optional structure -- faster for simple consultations.
