Overview

Digital consent forms replace paper-based consent workflows. Patients can sign consent forms on a tablet or phone, and the signed documents are stored in their medical record.

Go to Consent Forms and click 'New Consent':

  1. Select the patient
  2. Choose the consent type from your templates (General Consent, Procedure-specific, Anesthesia, etc.)
  3. The form auto-fills with patient details and procedure information
  4. Review the consent text with the patient
  5. The patient signs digitally on the screen (finger or stylus)
  6. Click 'Save' to store the signed consent

The signed consent is timestamped, linked to the patient's record, and available for audit at any time.

Configure consent templates from Settings > Clinical Templates > Consent Forms. Each template includes:

  • Title and description
  • Consent text with placeholders for patient name, procedure, and date
  • Witness fields (optional)
  • Multi-language support (English, Hindi, regional languages)

Common templates for small clinics:

  • General treatment consent
  • Minor procedure consent (wound suturing, incision & drainage)
  • Injection/vaccination consent
  • Photography consent (for dermatology, wound documentation)

Notes

Tip

Use an inexpensive Android tablet at the reception desk for patient consent signatures. The tablet can also be used for patient check-in.
