Overview

The Transfusion module tracks blood product administration to patients and monitors for adverse reactions. It creates a complete chain of custody from blood bank to bedside.

Transfusion Workflow

  1. Nurse collects the cross-matched unit from the blood bank
  2. At bedside, verify:
    • Patient identity (two-person verification)
    • Bag label matches the cross-match report
    • Blood group matches patient records
    • Expiry date is valid
  3. Start the transfusion and record:
    • Start time and initial vital signs
    • Flow rate
    • Monitoring vitals at 15 min, 30 min, 1 hour, and completion
  4. Record completion time and total volume transfused
  5. Monitor for 24 hours post-transfusion for delayed reactions

Transfusion Reactions

If a reaction occurs during or after transfusion:

  1. Stop the transfusion immediately
  2. Open the transfusion record and click 'Report Reaction'
  3. Document:
    • Type: Febrile, Allergic, Haemolytic, Anaphylactic, TRALI, TACO, other
    • Severity: Mild, Moderate, Severe, Fatal
    • Signs and symptoms
    • Actions taken
  4. The system alerts the blood bank and generates an incident report
  5. The remaining blood and patient samples are sent for investigation

Reaction reports feed into the quality metrics dashboard for trend analysis.

Notes

Warning

Two-person verification at bedside is mandatory before starting any transfusion. The system requires two staff members to digitally confirm the identity check.
