Overview
Clinical Pathway Templates standardize care delivery for common diagnoses and procedures. A pathway defines the expected sequence of interventions (medications, labs, nursing tasks, diet orders, milestones) across the patient's hospital stay, reducing variation and improving outcomes. Pathways are accessible at /dashboard/ipd/clinical-pathways and can be applied to any IPD admission.
Template Structure
Each pathway template consists of:
- Name & Category -- e.g., 'Total Knee Replacement', category 'Orthopedics'
- Condition / Diagnosis -- The target clinical condition
- Duration -- Expected length of stay in days
- Milestones -- Key checkpoints with expected completion day (e.g., Day 0: Pre-op assessment, Day 1: Surgery, Day 2: Mobilization, Day 4: Discharge readiness)
- Order Sets -- Pre-configured medication, lab, and nursing orders for each phase
- Variance Tracking -- When actual care deviates from the pathway, the system records the variance with reason codes
Templates are created by clinical leads and approved by the medical director before activation.
Applying a Pathway
To apply a clinical pathway to an admission:
- Navigate to the patient's IPD admission detail
- Open the Clinical Pathways tab (or go to
/dashboard/ipd/clinical-pathways) - Select a pathway template from the library
- Review and customize the milestones and order sets for this specific patient
- Activate the pathway -- this creates a pathway instance linked to the admission
- Track progress as milestones are completed or varied
Multiple pathways can be active simultaneously for patients with comorbidities.
Variance Management
When the actual care deviates from the expected pathway:
- The system detects overdue milestones (milestones not completed by their target day)
- Clinicians can mark a milestone as 'varied' with a reason code (e.g., patient condition changed, complications, patient refusal)
- Variance reports aggregate across all pathway instances for quality improvement analysis
- The pathway completion percentage is tracked in real-time on the admission detail page
Reporting
Pathway analytics include:
- Pathway compliance rate (% of milestones completed on time)
- Average length of stay vs. expected for each pathway
- Most common variance reasons by pathway and department
- Cost comparison: patients on pathway vs. off-pathway
- Readmission rates for pathway-managed patients

Tip
Start with high-volume, well-defined conditions (e.g., elective surgeries, normal deliveries) when building your first clinical pathways. These offer the highest ROI.
Warning
Clinical pathways are guidelines, not rigid protocols. Clinical judgment always takes precedence. The system records variances, not errors.
