Overview

Discharge Planning ensures a smooth transition from hospital to home. It begins at admission (estimated discharge date) and progresses through clinical readiness, discharge summary, billing clearance, and patient education.

Discharge Workflow

  1. Doctor marks the patient as 'Ready for Discharge' in the IPD console
  2. System triggers a discharge checklist:
    • Clinical clearance by attending doctor
    • Final diagnosis and procedure documentation
    • Discharge medications prescribed
    • Follow-up appointments scheduled
    • Discharge instructions documented
    • Insurance/TPA claim finalized (if applicable)
    • Bill reviewed and payments cleared
    • Nursing checklist: IV lines removed, wound care instructions given, belongings returned
  3. Doctor generates the Discharge Summary
  4. Patient/attendant signs acknowledgment
  5. Bed is released and marked 'Housekeeping Needed'
  6. System triggers automated bed sync to update availability
Discharge workflow

Discharge summary is finalized and printed or shared.

5Patient exit

Payment, documents, and follow-up advice are completed.

6Bed release

Bed status returns to housekeeping or available state.

Discharge Summary

The Discharge Summary is auto-generated with:

  • Patient demographics and admission details
  • Admission and discharge diagnosis (ICD-10 coded)
  • Hospital course and treatment given
  • Operative notes (if surgery was performed)
  • Investigation results summary
  • Condition at discharge
  • Discharge medications with instructions
  • Follow-up plan and warning signs
  • Doctor's signature

The summary can be printed or shared digitally. It is stored permanently in the patient's EMR.
