Overview

The Health Information Management (HIM) module handles medical records administration -- coding, indexing, deficiency tracking, release of information, and compliance with health data regulations. It bridges the gap between clinical documentation and administrative reporting.

Medical Coding

Assign standardized codes to diagnoses and procedures:

  • ICD-10-CM for diagnoses (auto-suggested from clinical notes)
  • ICD-10-PCS for inpatient procedures
  • CPT for outpatient procedures and services
  • SNOMED-CT for clinical terminology mapping

The coding workbench shows the clinical encounter alongside the coding interface. Coders can query the AI-assisted code suggestion engine which analyzes clinical text and recommends appropriate codes.

Deficiency Tracking

Track incomplete medical records:

  • Auto-detect missing signatures, unsigned orders, incomplete notes
  • Assign deficiencies to responsible physicians
  • Track aging of incomplete records
  • Send automated reminders with escalation to department heads
  • Dashboard showing deficiency rates by department and physician

Records are flagged as Complete, Deficient, or Delinquent based on configurable time thresholds.

Release of Information (ROI)

Manage requests for patient health information:

  • Log ROI requests from patients, insurers, legal entities, and other providers
  • Track consent and authorization status
  • Queue records for review and redaction
  • Generate cover letters and certifications
  • Log all disclosures for accounting of disclosures compliance
  • Turnaround time tracking with SLA alerts

Record Retention & Destruction

Manage the medical record lifecycle:

  • Configure retention policies by record type (e.g., 30 years for minors, 10 years for adults)
  • Auto-flag records eligible for destruction
  • Destruction workflow with multi-level approval
  • Certificate of destruction generation
  • Audit trail for all retention and destruction actions

Reports & Analytics

HIM-specific reports include:

  • Coding accuracy and productivity reports
  • Case Mix Index (CMI) analysis
  • Deficiency rate trends
  • ROI turnaround time metrics
  • Record completion rates by department
  • DRG (Diagnosis Related Group) analysis for reimbursement optimization

Notes

Tip

Use the AI code suggestion engine during discharge summary completion -- it can suggest ICD-10 codes from the clinical narrative, reducing coder workload.

Clinic tip

Small clinics without a dedicated HIM team should at minimum enable auto-coding during discharge and use the deficiency tracker for unsigned notes.
