Overview

The Gastroenterology module supports GI endoscopy documentation, liver disease management, and digestive disorder tracking. It provides structured reporting templates for upper and lower GI endoscopies, ERCP, and EUS, and integrates with endoscopy imaging systems for photo documentation.

The module includes two sub-pages: Endoscopies and Liver Clinic.

Endoscopy Documentation

The Endoscopy sub-module (Gastroenterology > Endoscopies) provides:

  • Pre-Procedure Checklist -- Patient preparation status (fasting, bowel prep, consent, sedation plan), anticoagulant management, and allergy documentation.
  • Procedure Reporting -- Structured templates for:
    • Upper GI Endoscopy (OGD): Esophagus, stomach, and duodenal findings with standardised descriptors (Los Angeles classification for esophagitis, Prague classification for Barrett's, Forrest classification for ulcers).
    • Colonoscopy: Segment-by-segment findings, polyp documentation (Paris classification, size, location, morphology), Boston Bowel Preparation Scale, cecal intubation time, and withdrawal time.
    • ERCP: Cannulation success, cholangiogram findings, sphincterotomy, stone extraction, stent placement.
    • EUS: Layer-by-layer assessment, lesion measurements, FNA documentation.
  • Photo/Video Capture -- Link endoscopy images and video clips to specific findings. Images are stored in the patient's record and can be included in the report.
  • Biopsy Tracking -- Document biopsy sites, number of samples, and link to histopathology results when available.
  • Post-Procedure Notes -- Recovery monitoring, complications, and follow-up plan.

Liver Disease Management

The Liver sub-module (Gastroenterology > Liver) includes:

  • Liver Disease Registry -- Classify patients by condition (Hepatitis B/C, NAFLD/NASH, Cirrhosis, Autoimmune, Wilson's disease).
  • Fibrosis Assessment -- Track FIB-4, APRI score, and Fibroscan (transient elastography) results over time.
  • Child-Pugh Score -- Auto-calculated from lab values (bilirubin, albumin, INR) and clinical parameters (ascites, encephalopathy grade).
  • MELD Score -- Auto-calculated for transplant listing priority. MELD-Na variant supported.
  • Hepatitis Treatment Tracking -- DAA regimen documentation for Hepatitis C, HBV viral load monitoring, treatment milestones (SVR12, SVR24).
  • Cirrhosis Surveillance -- Variceal screening schedule, HCC surveillance (AFP + USG every 6 months), ascites management, and SBP prophylaxis.
  • Transplant Work-up -- Pre-transplant evaluation checklist, donor assessment, and MELD exception documentation.

GI Symptom Tracking

Standardised symptom assessment tools:

  • GERD-Q (Gastroesophageal Reflux Questionnaire)
  • Bristol Stool Scale for bowel habit documentation
  • IBS Severity Scoring System (IBS-SSS)
  • Wexner Constipation Score
  • Dysphagia grading (FOIS scale)
  • Nutritional Risk Screening (NRS-2002)

Scores are stored longitudinally and plotted on trend charts for treatment response monitoring.

Clinic tip

For standalone endoscopy centres, focus on setting up the endoscopy reporting templates and biopsy tracking. These cover 95% of daily documentation.

Warning

Always document the Boston Bowel Preparation Scale for colonoscopies. Poor prep documentation is a common quality audit finding.
