Overview

The Nephrology module supports chronic kidney disease (CKD) staging, dialysis session management (haemodialysis and peritoneal dialysis), transplant recipient follow-up, and renal-specific lab tracking with eGFR trending.

CKD Staging & Monitoring

Automatic CKD staging based on eGFR (CKD-EPI formula) and albuminuria:

  • eGFR is auto-calculated whenever serum creatinine results arrive from the lab.
  • CKD stage (1-5) and risk category (low/moderate/high/very high) are displayed on the patient dashboard.
  • Trend charts track eGFR, creatinine, urea, potassium, calcium, phosphate, PTH, and haemoglobin over time.
  • Stage-appropriate care plan reminders (e.g., nephrology referral at Stage 3b, dialysis planning at Stage 4).

Haemodialysis Management

Manage HD sessions end-to-end:

  1. Schedule recurring HD sessions (e.g., MWF or TTS) with machine and shift allocation.
  2. Pre-dialysis assessment: Dry weight, interdialytic weight gain, vitals, access site inspection.
  3. Intra-dialysis monitoring: Hourly vitals, UF rate, blood flow rate, dialysate composition, heparin dosing.
  4. Post-dialysis assessment: Post-weight, vitals, access site status, Kt/V calculation.
  5. Monthly adequacy review: URR, Kt/V trending, anaemia management, mineral bone disease parameters.

The dialysis flowsheet is optimised for nurses to document quickly during sessions.

Peritoneal Dialysis

For PD patients:

  • CAPD exchange logging: Fill volume, dwell time, drain volume, effluent appearance for each exchange.
  • APD machine data import (where supported).
  • PET (Peritoneal Equilibration Test) recording and transport category classification.
  • Peritonitis episode documentation with culture results and antibiotic protocol tracking.
  • Monthly adequacy assessment: Weekly Kt/V, creatinine clearance, residual renal function.

Transplant Follow-Up

Post-transplant care tracking:

  • Immunosuppression medication management with drug levels (tacrolimus, cyclosporine, mycophenolate).
  • Graft function monitoring: Creatinine trends, protocol biopsy scheduling.
  • Rejection episode documentation: Biopsy findings (Banff classification), treatment protocol.
  • Infection screening schedule: CMV, BK virus, TB screening.
  • Long-term complication tracking: PTDM, cardiovascular risk, malignancy screening.

Vascular Access Management

Track dialysis access:

  • Access type: AVF, AVG, permanent catheter, temporary catheter.
  • Creation date, maturation assessment, first cannulation date.
  • Access complications: Stenosis, thrombosis, infection, aneurysm.
  • Surveillance: Flow measurements, access recirculation, physical examination findings.
  • Intervention history: Angioplasty, thrombectomy, revision, new access creation.
